Abstract

Parkinson’s plus syndrome is a rare entity, usually a group of heterogeneous degenerative neurological disorders, which differ from the classical idiopathic Parkinson’s disease in certain associated clinical presentations, poor response to levodopa, distinctive pathological characteristics and poor prognosis. Progressive supranuclear palsy (PSP), multiple system atrophy (MSA) and dementia with Lewy body disease (DLB) are commoner disorders. Differential diagnosis and their distinctions is challenging for florid mixed presentations and recent developments in neuroimaging helps provide great deal of insight in resolving these conundrums. We hereby present a rare case of 66 years old longstanding Parkinson’s who presented with overwhelming neuropsychiatric presentations with set of diagnostic challenges at our tertiary care centre.
